Dementia Care Program
Caring for a loved one with dementia is often very difficult, with family members and caregivers experiencing high levels of both physical and emotional stress. To alleviate some of this anxiety, we have created a specialized care plan for patients diagnosed with dementia, which includes support and education for caregivers.
Training provides tools to physicians, RNs, nursing assistants, social workers, chaplains, and volunteers who assist in increasing and maintaining a patient’s quality of life, improving pain and symptom management, and preparing Advance Care Planning.
Veteran Care Program
One out of every four individuals who are at the end of life in the United States is a veteran, and each veteran has unique needs at the end of life. We have the dedication and commitment to providing veteran-centered education to staff and volunteers to ensure that veterans have access to quality end-of-life care. We strive to address the unique healthcare needs of veterans, both physical and emotional, including the psychological toll of war and how it impacts their end-of-life journey.
